These codes look like a random mix of letters and numbers. They are designed to be confusing. If you do not understand the code, you cannot fight the denial.

The Two Types of Codes
To win your appeal, you must look for two different things on your paperwork:
- CARC Codes: These are Claim Adjustment Reason Codes. They tell you the "Financial" reason for the denial. (Example: The service is not covered by your plan.)
- RARC Codes: These are Remittance Advice Remark Codes. These give more detail. They tell you the "Specific" reason why they chose not to pay.
Why They Use Codes Instead of Words
Insurance companies use codes because it is faster for their computers. But it also creates a barrier to entry. Most people see a code like "CO-50" and give up.
When you "decode" the message, you take away their power. You move from being a confused patient to an informed authority.

Three Common Codes and What They Mean
- Code CO-50: This is the most common code for Medical Necessity. It means they think the treatment was not needed.
- Code CO-197: This means Pre-certification was missing. You were supposed to ask for permission before the treatment.
- Code CO-27: This means your coverage was not active on the day of the treatment.
How to Use the Code to Win
